1. Eligibility

You must be at least 18 years of age to use our services independently. Individuals under 18 may access certain services only with the presence and written consent of a parent or legal guardian.

2. Services

ELEVE Longevity Lounge provides wellness, longevity, and regenerative services including but not limited to: cryotherapy, infrared sauna, halotherapy, IV drip therapy, light therapy, PEMF, compression therapy, hydrogen therapy, dry float therapy, peptide therapy, hormone optimization, medical weight loss, and diagnostic testing.

Our services are not a substitute for professional medical advice, diagnosis, or treatment. Always consult with a qualified healthcare provider before beginning any new wellness program.

3. Bookings & Cancellations

• Appointments may be booked through our website, booking platform (Zenoti), or by phone.
• We require at least 24 hours' notice for cancellations or rescheduling. Late cancellations or no-shows may be subject to a fee.
• ELEVE reserves the right to cancel or reschedule appointments due to unforeseen circumstances, in which case you will be notified promptly.

4. Memberships

• Membership plans are subject to a minimum commitment period as specified at the time of enrollment (typically 6 months).
• Membership benefits are non-transferable and for use by the enrolled member only.
• Unused sessions or credits do not roll over to the following month unless otherwise stated in your membership agreement.
• Membership fees are billed monthly. You may cancel after your minimum commitment period with 30 days' written notice.

5. Payments & Pricing

• All prices are listed in US dollars and are subject to change without prior notice.
• Payment is due at the time of service or as outlined in your membership agreement.
• We accept major credit cards and other payment methods as indicated at our locations or on our booking platform.
• Promotional pricing and founder rates are subject to specific terms and availability.

6. Health Disclaimer & Assumption of Risk

By participating in any ELEVE services, you acknowledge that wellness and longevity treatments carry inherent risks. You agree to:

• Disclose all relevant medical conditions, medications, and health concerns prior to receiving any treatment.
• Follow all pre- and post-treatment instructions provided by our staff.
• Assume responsibility for any risks associated with the services you choose to receive.

ELEVE is not liable for adverse reactions or outcomes resulting from undisclosed medical conditions or failure to follow provided guidelines.

7. Intellectual Property

All content on our website and digital platforms — including text, images, logos, graphics, and branding — is the property of ELEVE Longevity Lounge LLC and is protected by applicable intellectual property laws. You may not reproduce, distribute, or use any content without our prior written consent.

8. Limitation of Liability

To the fullest extent permitted by law, ELEVE Longevity Lounge LLC shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services, website, or digital platforms.

These Terms of Service are governed by and construed in accordance with the laws of the State of California. Any disputes arising from these terms shall be resolved in the courts of Orange County, California.
